說明：

  1. 依本校105學年度上學期期初校務會議決議通過本學年度教評會委員總數為11人，除當然委員3人（校長、家長會代表、教師會代表）外，應選委員8人，候補委員4人；又依高級中等以下學校教師評審委員會設置辦法規定，任一性別委員應占委員總數三分之一以上（4人），委員未兼行政之教師不得少於委員總額之二分之一（6人）。
  2. 本校於105年8月26日校務會議辦理票選委員選舉，得票情形如附件，尾數同票者則公開抽籤決定。爰依上揭規定可得委員名單如下：（一）當然委員：林國源校長、家長會代表、教師會代表（蔡宏德組長）。（二）票選委員：羅崇源主任、詹勳超老師、張長壽主任、林如玉老師、江聖文老師、李芬珍老師、王頌榮老師、徐淑瑛老師。（三）候補委員：謝寶財主任、程正主任、楊秉正主任、方榮松老師、高士欽老師、翁嘉豪老師。

  3. 統計：

    選舉人／被選舉人： 49人／49人

    領票人／投票人： 46人／45人

    有效票／無效票：41票／4票
