一、 依據高級中等以下教育階段非學校型態實驗教育實施條例辦理。
二、 欲申請者，請檢具以下資料1份，於4月30日前寄（送）至本府教育處(地址：花蓮市達固湖灣大路1號)彙辦：

(一)

 申請書：學生為父母共同監護，法定代理人請父母2人同時簽名，申請書需請設籍學校核章。

(二)

 實驗教育計畫書及相關證明文件。
三、 國民教育階段欲申請者，其學籍設於原學區學校。
四、 旨揭條例及相關表件請參考附件。
