申請資格
  1. 學生本人或直系親屬領有身心障礙證明且家境清寒
  2. 前一學期學業總成績平均70分以上，且無任何一科不及格（若學生本人為身心障礙則免體育成績）

申請文件
  1. 學生自我概述(400字內且需親簽或蓋章)
  2. 身心障礙證正反面影本
  3. 上學年成績單（七年級須附小六成績單）
  4. 當年度清寒證明
  5. 全戶戶籍謄本（三個月內）
  6. 師長薦言單
