申請資格
  1. 在監服刑收容人之子女（113年仍在監）
  2. 未有重大不良操行紀錄者

申請文件
  1. 申請表
  2. 學生證影本（已蓋112-2註冊章）
  3. 證明與受刑人親屬關係之證明文件(如：戶口名簿影本)
  4. 申請人之父或母113年度開立在監(所)服刑證明文件正/影本乙份(在監證明最下方日期為113年X月X日)
  5. 郵局存摺影本（只接受郵局）
  6. 前一學期成績單
  7. 自傳或師長推薦函（擇一）
  8. 領款收據
  9. 若本人無郵局帳戶之代領切結書

其他說明
  1. 提供以下證明可加分
    1. 低收入戶、身障手冊、原住民身分、清寒證明等
    2. 112-113年間公共服務證明或優秀得獎紀錄
