每名4000元（國中組：30名）

申請資格
  1. 領有低收入戶/中低收入戶證明書 或 領有各縣市政府核發身心障礙手冊
  2. 前一學年(含上下學期)總平均70分以上

申請文件
  1. 申請表（如附件，老師將協助輸入電子資料）
  2. 上下學期成績證明書正本
  3. 學生證正反面影本
  4. 身分證或健保卡影本
  5. 低收入戶證明書正本 或 身心障礙手冊影本
  6. 六個月內全戶戶籍謄本或詳細記事版戶口名簿的影本
  7. 申請人之存摺封面影本（若申請人無個人銀行帳戶，則提供同戶籍直系血親之銀行帳戶存摺。）
