獎助金額

每名10000元，國中組60名

申請資格
  1. 正就讀國中，須為正式學制並具有學籍證明。
  2. 申請者本人或直系親屬(如：(外)祖父母、父、母罹患癌症且目前治療中或完成治療2年內。
  3. 112學年度成績平均分數達60分以上 或 具有特殊表現（如：體育、美術等優異成績）。

申請文件
  1. 報名表。
  2. 罹癌者之診斷證明書影本(須載明治療方式與治療計畫)。
  3. 三個月內之同住全戶人口戶籍謄本影本。
  4. 112學年度成績單，特殊表現請附證明。
  5. 113學年度學生證影本。
  6. 需檢附「自傳感想（500字以上）」
  7. 一年內的個人照兩張及家人合影生活照兩張。
  8. 相關證明文件(如：低收/中低收證明、身障手冊、醫療重症證明等)。
  9. 推薦函，請載明推薦人與推薦身分。(無則免附)
